As loosely agreed between @hansegucker and me, we should remove the model extensions mechanism.

For 4.x, we will remove the public (jsonstore-based) API (and with that, solve #440), and tell downstream app maintainers to migrate their extended fields into their own models.

For 5.x, we will remove extended_data.
